How to use this form:

  1. Fill in your details and list the cards you want graded.
  2. Read and tick the waiver and (optional) data privacy consent.
  3. Click Print / Save as PDF — save to your phone or print.
  4. Bring the form and cards to our Sta. Lucia branch, or complete payment first then ship your cards to:

C · Terms & waiver
  1. Declared value & protection. While my card(s) are in the collectible store’s custody, its liability is limited to the declared value, up to ₱60,000 per card. Any reimbursement is based on the card’s verified market value at the time of the incident, not the declared amount alone.
  2. Grading outcome. Grades are determined solely by TAG and are not refundable. the collectible store does not predict or guarantee grades.
  3. Transit & grading at TAG. Handling during international transit and at TAG is governed by TAG’s own guarantee and insurance.
  4. Timeframes. Turnaround times are estimates and may be affected by TAG’s queue, customs, and couriers.
  5. Events beyond control. the collectible store is not liable for loss or damage caused by events beyond its reasonable control.
  6. Authorization. I authorize the collectible store to register a TAG account in my name and submit my card(s) to TAG on my behalf.

By checking, I consent to the collection of my personal information for promotional and marketing communications from the collectible store. I understand that:

  • My information will be used strictly for promotional and marketing purposes.
  • My data will be used to create a TAG account on my behalf, governed by TAG’s own privacy policy.
  • My data is handled in accordance with RA 10173 (Data Privacy Act of 2012).
  • I may withdraw consent at any time by contacting tag@herohubventures.com.
  • My data will not be shared with unauthorized third parties.
